Mechanism of medicine action:
Cabergoline is a long-acting dopamine D2 receptor agonist that suppresses pituitary lactotroph activity. By activating D2 receptors, it inhibits adenylate cyclase, lowers cAMP, and decreases prolactin gene expression and secretion, leading to dose-dependent reductions in serum prolactin and restoration of gonadotropin pulsatility. Its prolonged effect reflects a long half-life and strong receptor binding. In prolactinomas, cabergoline reduces hypersecretion and can induce tumor shrinkage via antiproliferative and pro-apoptotic actions on lactotroph cells, improving clinical manifestations of hyperprolactinemia.
Side effects:
Cabergoline may cause nausea, abdominal discomfort, constipation, headache, dizziness, and orthostatic hypotension, especially after dose initiation or escalation. Central effects include somnolence, insomnia, agitation, and, rarely, impulse-control disorders (e.g., gambling, hypersexuality). Reduced prolactin can modify libido and reproductive function. Less commonly, edema, nasal congestion, and vasospasm occur. With higher cumulative doses or prolonged use, ergot-derived dopamine agonists have been associated with valvular heart disease. Concomitant dopaminergic or hypotensive agents may increase neuropsychiatric effects or hypotension.
Dosage:
Take 125-500 mcg orally once or twice a week.
